Dr. Hays participates in most major insurance plans, Worker's Compensation, and No Fault insurance.  Please contact our office with any questions regarding coverage.  When you arrive for your appointment, please have your insurance card and information available.  If your insurance requires that you have a referral from your primary care physician or pre-authorization, you should obtain this prior to your scheduled appointment.  If you do not have the required items, your evaluation may be delayed, and you may be asked to reschedule your appointment.
